When Your Smile Isn’t Aging as Gracefully as You Are

You might have been one of the lucky few born with perfectly straight teeth and a healthy bite. You might have spent months in orthodontic treatment as a teenager to achieve perfectly straight teeth and a healthy bite. But now that you’re growing older, you might be unhappily surprised to discover that your smile isn’t aging as gracefully as you are.  What’s changed?

That’s a trick question, because our bodies never stop changing, growing, and adapting. And these constant adjustments include the changes taking place in your teeth and mouth. You might begin to notice subtle differences in your smile when you’re in your thirties or forties. After young adulthood, several factors come into play which can cause shifting teeth and a misaligned bite:

  • Teeth naturally shift.

Shifting can be a result of the normal changes time brings. The periodontal ligaments which attach our teeth firmly to the jawbone lose some of their strength; the jawbones which hold our teeth in place lose some of their density and begin to narrow. Our teeth also have a natural tendency to move toward the front of the mouth, a phenomenon called “mesial drift.”

Add all of these elements together, and your once straight teeth start to crowd together and even overlap—especially the front bottom teeth.

  • Stressful habits stress your teeth.

If you habitually grind or clench your teeth, you’re putting pressure on them. Just like the gentle pressure of braces and aligners can shift teeth into alignment, the more uncontrolled force of grinding can push teeth out of alignment.  

  • Losing a tooth affects surrounding teeth.

Nature abhors a vacuum, and so does your smile. If you lose a tooth, your other teeth will automatically start to drift into the space left open by the missing tooth.

  • A neglected retainer is gathering dust in a drawer somewhere.

You might have spent time as a teenager in orthodontic treatment, with a beautiful smile to show for all your hard work. And, back in the day, your orthodontist no doubt let you know that you needed to keep wearing your retainer at night once your treatment was completed.

Your beaming smile makes a sunny day even brighter—and that sunny day can return the favor! How does the sun affect your dental health? Let’s shine some light on the subject.

You may have heard how important calcium is to your body. Over 99% of the calcium in our bodies is used to form teeth and bones, providing strength and structure to the hardest parts of the body. But that’s not the only task on the calcium to-do list. Calcium is also necessary for:

  • muscles to work
  • nerves to carry messages
  • blood to deliver nutrients and oxygen
  • the release of hormones

When we don’t get enough calcium in our diets, our bodies start to draw the calcium they need for these critical functions from our teeth and bones. Over time, a severe calcium deficiency can lead to teeth that are brittle, or more vulnerable to decay, or loose due to weakened bone density in the jaw.

Keeping up with your calcium is especially important when you’re wearing braces or aligners. Calcium helps protect you from tooth decay and gum disease, which could delay your treatment. And you want your jaw bones strong and healthy as the bone tissue reforms and then stabilizes around your teeth as they move to their new, ideal positions.

We can get the calcium we need from a balanced diet. Dairy products are our most common source of calcium, and several servings of milk, cheese, or yogurt each day supply most of our needs. If dairy’s not on your menu, though, you can get calcium from other foods like broccoli, kale, collard greens, canned salmon, or sardines. Because calcium is so essential to our health, many foods are fortified with it, including fortified juices, bread, cereals—even tofu!

But there’s a snag here, and it’s a big one. We can’t really benefit from our careful consumption of calcium without the help of vitamin D. If calcium is the doorway to strong teeth and bones, vitamin D is the key which unlocks all of calcium’s benefits.

Without enough vitamin D, our bodies can’t absorb and make use of calcium. However, getting enough of this essential vitamin can be a problem. While we can get most of the vitamins we need from a balanced diet, there aren’t a lot of foods which are naturally rich in vitamin D.

So, how do we get this vital vitamin? The original option comes from nature.

  • Sunlight

Vitamin D is called “the sunshine vitamin” because of the unique chemical reaction which creates it. When the ultraviolet B rays of the sun reach special cells in our skin, they convert cholesterol in these cells to a basic form of vitamin D. These vitamin molecules travel through the body, picking up hydrogen and oxygen molecules in the liver and kidneys to become active vitamin D.

Since sunlight exposure is easy, available, and free, it seems like a simple solution to getting the vitamin D we need.

But we’ve understandably moved away from the days of basking in the sun for hours at a time. Over-exposure to UV rays damages the skin, causing premature aging and, worse, a much greater risk of skin cancer. If you enjoy time in the sun, talk to your doctor about safe sun exposure, which can depend on the part of the country you live in, the time of year, and your individual skin type.

And, if you need to avoid sun exposure, there are other opportunities for getting the vitamin D you need.

  • Diet

While there aren’t a lot of foods which are naturally rich in vitamin D, there are still a few options on the menu. Oily fish like salmon, tuna, and herring—and old-fashioned cod liver oil—are good sources of the vitamin. But most of us don’t live on a strict fish diet!

Instead, we can get the vitamin D we need from fortified foods. Vitamin D is commonly added to cow’s milk, soy milk, orange juice, and cereals to make sure the calcium in our diet is working effectively to keep our teeth and bones strong. Check out nutritional labels to discover whether your favorite grocery products contain vitamin D. And don’t forget to choose foods that are braces-friendly, as well!

  • Supplements

For hundreds of years, people who lived in climates and cities without a lot of sunshine suffered from the effects of vitamin D deficiency. Diseases like rickets, where the teeth and bones don’t form properly, were common in children. Adults who couldn’t absorb enough calcium suffered from weak teeth and bones.

Today, depending on age, allergies to sun or food products, dietary choices, and health conditions, some people are still at risk for vitamin D deficiency. If you’ve been diagnosed with this deficiency, your doctor might recommend a vitamin D supplement. (It’s important to take any supplement as directed, because taking too much vitamin D can be harmful, too.)

So, which form of vitamin D is best? Fortified foods? Supplements? Or is it that radiant original source—the sun? No need to worry. When Clear Aligners aren’t the Answer

Clear aligners like Invisalign® have become increasingly popular over the past several years and rightly so. They’re removable, easier to clean than braces, and hardly anyone knows you're wearing them. They are great in treating many cases, but they aren't for everyone.

Below, Drs. Cartsos and Zavras and our team cover some of the instances where clear aligners just aren't the answer:

  • If drastic tooth movement is required – Fixed appliances deliver much more significant tooth movement. So if your case is a drastic one, clear aligners may not be the best choice.
  • If you need to move molars – Molars have much stronger roots than your other teeth and would require significantly longer to move with clear aligners. A fixed appliance is the best choice in this instance, especially if you have a substantial overbite or underbite that needs to be dealt with.
  • If you're the type who often forgets or loses things –If you would forget to wear your aligners for the prescribed amount of time (usually at least 22 hours per day), clear aligners are probably not the best choice for you. Forgetting to wear them can delay treatment and even make it so you need to regress to the previous set of aligners to be able to move forward with treatment. And let's face it, if you're not careful, removable aligners are easy to lose. Losing aligners delays treatment and is expensive since you need to buy replacements to stay on course. Replacing a lost set of aligners usually takes between seven and ten days—a definite setback in treatment.
  • If you're looking for the fastest treatment possible – Clear aligners usually can't move teeth as quickly as fixed appliances. So if you're looking for the fastest way to achieve your desired result, clear aligners may not be the best bet.

Working Behind the Scenes—Lingual Braces

There are many great reasons to see an orthodontist. For a healthier bite. For straighter teeth. For a more confident smile. So why are you hesitating? If the visibility of traditional braces is what’s holding you back, ask Drs. Cartsos and Zavras about lingual braces.

With regular braces, brackets are bonded to the front of each tooth with a special adhesive. Ligatures around each bracket or bracket clips grip an archwire, which does the work of moving the teeth. The gentle pressure from the wire guides the teeth into alignment in gradual stages. Every adjustment moves the teeth to their perfect positions. These braces are quite effective—and they are usually quite visible.

Lingual braces, on the other hand, are virtually invisible. Lingual means “toward the tongue,” and this placement is the difference between lingual braces and more traditional types of orthodontic braces.

Lingual braces are custom designed to be applied to the inside of your teeth. Specially designed brackets are attached to the backs of the teeth. Individually crafted archwires are used to guide your teeth to their best alignment.

Lingual braces can be the solution to many orthodontic concerns:

  • If you need or want invisible braces for personal or professional reasons, lingual braces are a great option. Because they are behind your teeth, they are even less noticeable than clear aligners—and you don’t need to keep track of your hours wearing them.
  • Lingual braces keep the front of your teeth braces-free for playing a brass or reed instrument, or for participating in sports. (Just remember, a mouthguard is always a good idea for athletic activities, and especially when you wear braces.)
  • Both brackets and wires can be customized to fit your teeth perfectly, and new lingual brackets and wires are more comfortable than ever.

You might be a good candidate for lingual braces if:

  • You have a large enough tooth surface to place a bracket. Adults with small teeth—or children—might not be have enough room on the back of each tooth to hold a bracket.
  • You don’t have a major malocclusion (bite problem) which would make lingual braces impractical. A deep overbite, for example, could cause the wires and brackets behind the upper teeth to come loose or detach as they come in contact with lower teeth.
  • You are dedicated to keeping up with your oral hygiene. Because wires and brackets are behind the teeth, it can be harder to keep them free from food particles and plaque.

Finally, even if lingual braces aren’t the perfect match for your orthodontic needs, there are other options that can work for you. Smaller metal brackets, ceramic brackets that blend in with your enamel, and clear aligners mean today’s orthodontic work is more subtle and discreet than ever before.
